Medical Community: How to Put Precision Medicine into Practice? Let’s Listen to the Two Academicians

On January 19, 2018, China-Japan Friendship Hospital(CJFH) held "Internet Plus Precise Medicine" Summit Forum and China Precision Medicine Integrated Demonstration System Construction Project Seminar. Academician Wang Chen, CJFH President said in his speech: "Nowadays, precision medicine in China is generally more conceptually talked about than actually used and we are going to change this situation now."

In 2017, CJFH led the national key research and development special project “Precision medicine integrated application demonstration system construction”. This forum is the first high-level meeting covering all the tertiary hospitals nationwide held by CJFH in precision medicine area since the initiation of “Precision medicine integrated application demonstration system construction” project. Before this, CJFH led and established Chinese Precision Medicine Clinical Research and Application Union, founded 1600 cooperating hospital network in actual operation nationwide, compiled Precision Medicine: Medication Outline instructive technical manual and established CJFH precision medicine model.

According to Academician Wang Chen, CJFH is steadily improving the establishment of precision medicine demonstration system and the integration of platform and resource now. It is hoped that the standardization of precision medicine could be pushed forward and the transform from definition to practice of precision medicine could be promoted to truly put precision medicine into practice.

At the forum, Professor Zhan Qimin, Academician of the Chinese Academy of Engineering and Vice President of Peking University combined technology innovation with precision medicine to carry out discussion. He said that the concept and idea of health has changed a lot. The diagnosis and treatment of diseases are viewed as the center previously and the all-round and full-cycle of life are more emphasized and concentrated today. However, our whole country still faces many challenges in health at present.

Academician Zhan Qimin listed some data of major diseases:

According to the latest data released by National Cancer Institute, the number of newly diagnosed tumors in China is 4.3 million each year with a death number of 2.8 million. Among these, lung cancer, gastric cancer, liver cancer and esophageal cancer rank the first in the world.

In addition, cardiovascular and cerebrovascular diseases cause more than 3.8 million deaths annually, about 2 million of cerebrovascular diseases and about 1.8 million of cardiovascular diseases. In China, there are 180 million people with hypertension, 170 million with hyperlipidemia, 110 million with diabetes and 150 million pre-diabetes population. There are more than 100 million chronic kidney disease patients and 12 million Alzheimer's patients with a new increase of 300,000 people each year. The number of people with birth defects adds 0.9 to 1.1 million every year and the total amount is over 20 million.

Apart from major chronic diseases, China is a country with large population and a country with large number of hepatitis patients at the same time. 80 million people are hepatitis B virus carriers in China and some of them will develop into liver cirrhosis or liver cancer. China has 5.5 million active tuberculosis patients, ranking the second in the world. In addition, the prevention and control task of unexpected infectious diseases is also very serious.

Corresponding to the severe health status, the medical service quality of our country needs to be greatly improved. China ranks the 82nd in the assessment of 195 countries in the world. However, among the 44 million poor people in China, 50% are related to illness-caused poverty. And in extremely poor areas, 80% of the poverty is caused by illness.

Academician Zhan Qimin also pointed out that 95% of the initial patents of clinical drugs (western medicine) in our country were from abroad, 95% of large-scale medical equipment is imported from foreign countries, and 95% of the clinical standards guidelines were borrowed from abroad. Our international contribution of cancer is less than 1%, while our international contribution of cardiovascular diseases is about 3.8%. In general, China contributes less than 5% to the international diseases standardized diagnosis and treatment guidelines.

"All the above problems and challenges show that today’s China is not yet a medical power or a great disease prevention and control power. The means to solve these problems is technological innovation. Long-term medical practice has proved that any mature application of a new technology, a new equipment and a new drug in disease prevention and control as well as clinical diagnosis and treatment is the result of medical science and technology development. "Academician Zhan Qimin said," medical science and technology innovation will generate revolutionary change to disease diagnosis methods, disease classification, clinical diagnosis and treatment pathway, norms, guidelines and standards and promote a number of new-type medical health industries’ development to achieve the corner overtaking of Chinese medicine.

Academician Zhan Qimin thought that from the historical evolution of medicine development, precision was the necessary requirement of its own development and a great demand of public health. The requirement of precision runs through the whole medical process including precise diagnosis of diseases, precise application of drugs, accurate assessment of the efficiency and the accurate prediction of the prognosis. The realization of precision needs the frontier multidisciplinary cross-integration including life omics technology, stem cells and regenerative medicine, vaccines and antibodies, biotherapies, personalized diagnostics, medical equipment, big data and intelligent medicine, all regarding technology innovation as its core.

"In the developing process of precision medicine, what we can do today is to decide whether China can take up a certain position in the new standard or just to be led by the West and learn from them.” Academician Zhan Qimin said: "Among many tracks of precision medicine, we are at the same starting line with the West and even with more advantages. It will be much helpful to well utilize our advantages. "
